Job Description
A health services organization is seeking a licensed Registered Nurse (RN) in the Bronx, NY. This role supports clinical care, care coordination, and health education for individuals served in a clinic-based setting.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday
- Hours: 9am to 4:30pm
- Setting: Clinic-based
Responsibilities:
- Coordinate and follow through on health care services as part of an interdisciplinary team
- Arrange medical services, make referrals, and provide follow-up on medical concerns
- Support care management needs through assessment, documentation, and monitoring of treatment plans
- Provide medication administration oversight, first aid, and emergency response as needed
- Conduct staff training and maintain compliance with infection control and regulatory requirements
Qualifications:
- At least 2 years of experience as a Registered Nurse in community-based health services
- Graduate of an accredited professional nursing program with a BSN
- Current New York State Registered Nurse license and registration
- CPR certification
Desired Skills:
- Experience working with individuals with intellectual and developmental disabilities
- Knowledge of adaptive equipment and related support needs
- Strong organizational, communication, and interpersonal skills
- Experience with care management and clinical documentation
