LifeBridge Health is seeking a compassionate and dedicated Registered Nurse to join our Healthcare & Medical Services department within our regional healthcare organization in Maryland. As a key member of an interdisciplinary care team, you will provide evidence-based, patient-centered nursing care to diverse populations across a continuum of inpatient and/or outpatient settings. In this role, you will perform comprehensive assessments, interpret clinical data, and develop individualized plans of care that reflect each patient’s unique needs, preferences, and goals. You will administer medications and treatments safely, monitor patient responses, and promptly address changes in condition while maintaining meticulous electronic health record documentation in alignment with organizational and regulatory standards. Collaboration is central to this position; you will work closely with physicians, advanced practice providers, therapists, case managers, and support staff to coordinate care and support smooth transitions across the continuum, including discharge planning and community referrals. Education and advocacy are also core responsibilities, as you will provide clear, compassionate instruction to patients and families about diagnoses, treatment options, medications, and self-care strategies that promote health and prevent readmission.
