  • Position Type: Contract
  • Contract Duration: 3 months
  • Pay Rate: $77.00 / Hour (USD)
  • Shift / Schedule: Day Shift, 5x8 Hours (8:00 AM 5:00 PM)

Job Summary

This Outpatient RN Care Manager role is based at a licensed Adult Day Health program in Napa, CA, serving frail elderly individuals, younger disabled adults, and people with Alzheimer's Disease and related dementia. The program is licensed for 100 participants per day with a current average daily census of 72. The RN role provides chronic disease case management, monitors medical conditions in conjunction with the Interdisciplinary Team (IDT) and Primary Care Provider (PCP) in the community, and provides caregiver education and support. The goal of the program is to reduce emergency department visits and hospitalizations, as well as prevent premature nursing home placement. The EMR system is Turbo TAR, which is specific to the California ADHC industry.

Key Responsibilities

  • Provide chronic disease case management for adult day health program participants
  • Monitor medical conditions in conjunction with the Interdisciplinary Team (IDT) and Primary Care Provider (PCP) in the community
  • Provide caregiver education and support
  • Work toward program goals of reducing ED visits and hospitalizations and preventing premature nursing home placement
  • Maintain a daily patient ratio of 1:40

Education Requirements

  • BLS certification required
  • CA RN License required at time of start (Pending CA License accepted)

Required Experience

  • Minimum 1 year of prior RN experience required
  • Experience in Clinic, Primary Care, Pre/Post, or Home Health setting(s) preferred

Required Skills

  • Proficiency with or ability to learn Turbo TAR EMR (specific to CA ADHC industry)
  • Reliable personal transportation required
  • No additional scheduling restrictions

  • Locals accepted; considered "local" within a 50-mile radius and will be accepted at the same rate
  • RTO restrictions: Confirm with hiring manager
  • Guaranteed Hours: Facility may call off traveler up to 1 shift per pay period (every 2 weeks)
  • First-time travelers accepted
  • COVID-19 vaccination: Not required
