    Summary

    Registered Nurse - Palliative Care (Full-Time)

    Location: Outpatient | Serving Carroll County, Baltimore County, and Baltimore City

    Organization: BridgingLife Hospice, part of LifeBridge Health

    About the Role:

    BridgingLife Hospice is seeking a compassionate, experienced Registered Nurse (RN) to join our Palliative Care team. This full-time position provides vital outpatient support to patients in Carroll County, Baltimore County, and Baltimore City. The RN will play a key role in coordinating and delivering comprehensive palliative care services that focus on comfort, dignity, and quality of life.

    Key Responsibilities:

    • Deliver and document medically necessary nursing services in accordance with provider orders.

    • Assess patient and family/caregiver needs; coordinate appropriate services (e.g., DME, home health care, hospice).

    • Collaborate with the palliative care provider to develop individualized plans of care aligned with treatment goals.

    • Provide disease management education and guidance to patients and families.

    • Conduct comprehensive nursing assessments, addressing emotional, physical, spiritual, and social needs.

    • Collaborate with the interdisciplinary team to ensure integrated, patient-centered care.

    • Support the development and application of clinical practice standards to ensure high-quality care.

    • Maintain compliance with all federal, state, and local regulations, as well as BridgingLife and LifeBridge Health policies.

    • Perform other related duties as assigned.

    Qualifications:

    • Active Registered Nurse (RN) license in Maryland (MBON required)

    • Current AHA Basic Life Support (BLS) certification required

    • Must meet Fleet Safety requirements

    • Prior experience in palliative care or hospice is preferred

    • Strong interpersonal, assessment, and organizational skills
