The part-time (19 hours/week) Registered Nurse provides assessment, education, and support to children and their families enrolled in the DDI Vantage Early Intervention Program.
Responsibilities:
- Conducts physicals, developmental evaluations, and initial/annual health assessments to assist in determining program eligibility on all referrals.
- Provides hearing and vision screening per Baby Watch Early Intervention program guidelines and completes required reports.
- Reads medical records and highlights relevant information for service providers in a timely manner.
- Possesses understanding and knowledge of a child's overall development and seeks consultation from other disciplines when appropriate.
- Collaborates with related disciplines to ensure appropriate service delivery for each child.
- Assists in identifying social service needs for families, when appropriate, and refers them to the program coordinator for help in accessing community resources.
- Assists parents and caregivers in their role as a child advocate when appropriate.
- Monitors and ensures the proper maintenance of medical equipment.
- Monitors, requisitions, and stocks medical supplies.
- Attends all mandatory CSPD training for early intervention credentialing under Part C rules.
- Assists in child-find activities.
- Participates in staff in-service training, serves on committees, attends workshops and conferences as requested or approved, and may provide in-service training to staff.
- Attends team meetings as necessary and all mandatory DDI Vantage meetings.
- Demonstrates professionalism in all aspects of the job.
- Safeguards the confidentiality of all clients, at all times.
- Completes written documentation in a timely manner to ensure program compliance.
- Fulfills other duties as assigned.
