Registered Nurse (RN) – Per Diem

Job Summary


Grace Senior Care is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our Adult Medical Day Care team on a per diem basis. The Registered Nurse is responsible for providing high-quality nursing care, monitoring participants' health status, coordinating with physicians and caregivers, maintaining accurate clinical documentation, and ensuring compliance with New Jersey Department of Health regulations.


This position offers flexibility and the opportunity to make a meaningful impact in the lives of older adults and individuals with disabilities in a supportive, team-oriented environment.


Responsibilities


  • Perform nursing assessments and ongoing participant evaluations.
  • Monitor participants' medical conditions and report significant changes to physicians and family members as appropriate.
  • Administer medications and treatments in accordance with physician orders and facility policies.
  • Maintain accurate clinical documentation and participant records.
  • Develop and update individualized care plans.
  • Respond to medical emergencies and provide appropriate nursing interventions.
  • Monitor infection prevention and control practices.
  • Educate participants and caregivers regarding health conditions and wellness.
  • Collaborate with physicians, social workers, therapists, activity staff, and other interdisciplinary team members.
  • Ensure compliance with New Jersey Department of Health regulations, HIPAA, OSHA, and facility policies.
  • Participate in quality improvement activities and staff meetings as needed.
  • Perform other duties assigned by the Director of Nursing or Administrator.
Qualifications


Required


  • Current New Jersey Registered Nurse (RN) License in good standing.
  • Current CPR/BLS Certification.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Ability to maintain participant confidentiality.
Preferred


  • Experience in Adult Medical Day Care, Adult Day Health Services, geriatrics, rehabilitation, long-term care, home health, or medical-surgical nursing.
  • Experience with electronic medical records (EMR).
Physical Requirements


  • Ability to stand and walk for extended periods.
  • Ability to assist participants with mobility when necessary.
  • Ability to lift up to 25 pounds with or without reasonable accommodation.
Schedule


  • Per Diem
  • Day Shift
  • Monday–Friday
  • No weekends
  • No overnight shifts
  • Hours based on facility needs
