    Registered Nurse- Premier Family Medicine (Full-time)- Days - Howell

    Full-time Benefit Eligible

    Schedule: Monday-Friday 8a-4:30pm; No weekend or holiday requirement

    Henry Ford Premier Family Medicine is a collaborative ambulatory family medicine practice dedicated to preventive care, chronic disease management, and improving the overall health of our community's Medicare population. Our Medicare Wellness Program plays a key role in helping patients stay actively engaged in their healthcare through annual wellness visits, preventive screenings, and coordinated care planning.

    The Medicare Wellness Nurse serves as a vital member of the care team, conducting Annual Medicare Wellness Visits, completing assessments, obtaining vital signs, administering injections, and identifying care gaps that support better patient outcomes. This role works closely with providers to communicate clinical concerns, coordinate follow-up care, and ensure patients receive recommended preventive services. Patient outreach and education are also important components of the position, helping eligible patients schedule and complete their wellness visits.

    While the nurse's primary home base is Premier Family Medicine, the position also supports Howell Family Medicine and Byron Medical Group. All three family medicine practices are conveniently located within the same building, allowing the nurse to collaborate with multiple provider teams and serve a diverse Medicare patient population without extensive travel between locations.

    GENERAL SUMMARY:

    Provides leadership in the coordination and delivery of quality compassionate patient care. Utilizes the nursing process of assessment, planning, implementation, and evaluation to provide, delegate, supervise and document care and teaching of patients and family. Effectively communicates with the health care team. Demonstrates clinical competence. May require a professional certification which is a regulatory requirement in the State of Michigan or a documented community standard for the position, works independently and in collaboration with principal investigator when appropriate.

    EDUCATION/EXPERIENCE REQUIRED:

    • Graduate from an accredited school of nursing.
    • Bachelor's Degree preferred.
    • Successful completion of Henry Ford Ambulatory orientation.
    • Maintains and demonstrates current clinical knowledge in area of specialty.
    • Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
    • Ability to perform a wide variety of professional nursing services.
    • Ability to effectively communicate with patients, families, and medical group personnel.
    • Preferred experience in the specialty. Basic computer knowledge and keyboarding skills.

    CERTIFICATIONS/LICENSURES REQUIRED:

    • Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
    • Current BLS or ACLS per Henry Ford Health policy.
