Description:
• This position involves providing primary care in a multi-specialty setting. 
• The role is contract-based, located in Flatbush, NY, and requires a commitment of 40 hours per week, Monday through Friday. 
• The Registered Nurse will work onsite, participating in patient assessments, care planning, and medical evaluations.

Requirements:
• Review and develop patient care plans based on needs and organizational policies.
• Support the implementation of organizational programs and procedures.
• Collect and adjust data to meet program requirements.
• Teach and support patients in self-care management.
• Follow up on abnormal lab results and manage medication refill requests.
• Coordinate outpatient work-up and treatment plans as needed.
• Provide direct care for patients requiring specific assessments.

Qualifications:
• Completion of nursing training from an accredited school of nursing.
• Minimum of one to two years' experience in a medical practice, primary care, med-surg, ER, or occupational health.
• Current New York State Registered Nurse license.
