The Virtual Care Center Registered Nurse (RN) plays a crucial role and is the heart and soul of our company. The Virtual Care Center RN serves as a frontline resource for oncology patients enrolled in ReimagineCare’sremotemonitoringand virtualcareprograms.Operatingwithinacollaborative, nurse-led model, the Virtual Care Center RN delivers evidence-based triage, symptom assessment, carecoordination,andpatienteducationviaSMSmessaging,telephone,andtelehealthplatforms. This role requires strong oncology clinical knowledge, independent judgment, and a commitment to compassionate, patient-centered care.
The primary schedule is Monday through Friday during daytime hours. In addition, this role participates in anintermittentrotatingweekendandovernighton-callpoolsharedamongtheVCC nursingteam.
On-call assignments are escalation-driven and triggered by patient urgent needs or clinical symptoms

Virtual Care Center Clinical Practice- Deliver direct patient care through the Virtual Care Center, including symptom assessment, triage,education,andcarecoordinationforoncologypatients
- Review patient-reported outcomes and physiologic monitoring data and support escalation pathways
- Work collaboratively with other members of the Virtual Care Center to follow all Reimagine Care policies, procedures, escalation pathways, and communication and documentation best practices.
- Bring frontline insightsback into program design,workflowimprovements,andproduct requirements
- Support quality reviews, audits, and identification of safety improvement opportunities
Qualifications & Experience
- Oncology Registered Nurse (RN) with a Bachelor of Science in Nursing
- At least 5 years of oncology nursing experience
- Preferred OCN Certification
- Experience with remote monitoring, virtual care, or care management programs preferred
- Strong organizational, communication, and documentation skills
- Demonstratesexceptionalcommunicationandinterpersonalskillswiththeabilitytobuild trust and therapeutic rapport with patients, caregivers, and oncology provider clinical teams
- Proficiency with Electronic Medical Records (EMR) required; experience with Epic, OncEMR, and/orCernerpreferred. Must be able to navigate multiple platforms simultaneously in a fast-pacedvirtualtriageenvironment.
