Job Title: Registered Nurse (RN) – Admission Assessment (PRN / Per Diem)


Employment Type: PRN - As Needed


Compensation: $30–$50 per completed assessment



Position Summary


The Registered Nurse (RN) – Admission Assessment is responsible for conducting comprehensive pre-service, in-home clinical evaluations for prospective clients prior to the initiation of care.



This PRN position plays a critical role in establishing safe, appropriate, and individualized care plans based on a thorough assessment of the client’s medical condition, functional status, and home environment. The RN ensures compliance with regulatory standards, agency policies, and evidence-based clinical practice.



Essential Duties and Responsibilities



  • Perform comprehensive in-home admission assessments for new clients
  • Obtain and review medical history, current diagnoses, medication profiles, and treatment plans
  • Conduct physical, functional, cognitive, and psychosocial evaluations
  • Assess home safety and environmental risk factors
  • Identify skilled and non-skilled service needs
  • Develop and document individualized care plans in accordance with physician orders and agency protocols
  • Collaborate with interdisciplinary team members to ensure appropriate service coordination
  • Ensure timely and accurate completion of documentation within established compliance standards
  • Maintain adherence to federal, state, and local regulations governing home health services
  • Protect patient confidentiality in accordance with HIPAA guidelines

Minimum Qualifications



Active, Registered Nurse (RN) license in the state of North Carolina



  • Minimum two (2) years of clinical nursing experience
  • Demonstrated proficiency in patient assessment and care planning
  • Strong clinical judgment and critical thinking skills
  • Excellent written and verbal communication abilities
  • Reliable transportation and ability to conduct home visits
  • Current CPR certification


Preferred Qualifications:


  • Prior home health, hospice, or community-based nursing experience
  • Knowledge of state home care regulatory requirements
