Base Pay: $31.00-$40.40 per hour (based on experience)

Glen Haven Health & Rehabilitation is seeking an organized, compassionate, and detail-oriented Registered Nurse (RN) Admissions Nurse to join our nursing team.

As the RN Admissions Nurse, you'll play an important role in welcoming new residents and helping ensure a smooth transition into our skilled nursing and rehabilitation community. You'll work closely with residents, families, nursing staff, physicians, and other members of the interdisciplinary team to coordinate the clinical admission process and establish a strong plan of care from day one.

RN Admissions Nurse Responsibilities

• Complete nursing assessments and clinical documentation for new admissions.
• Review hospital discharge records, physician orders, medications, and other clinical information.
• Coordinate the nursing admission process and communicate resident needs to the care team.
• Ensure physician orders and required admission documentation are accurate and complete.
• Communicate with residents and families throughout the admission process.
• Collaborate with nursing, therapy, physicians, pharmacy, and other interdisciplinary team members.
• Identify clinical concerns and ensure appropriate follow-up.

Qualifications

• Current Alabama Registered Nurse (RN) license in good standing – REQUIRED.
• Current CPR/BLS certification.
• Previous nursing experience required.
• Skilled nursing, rehabilitation, long-term care, post-acute care, or admissions nursing experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication, organization, and attention to detail.
• Experience with electronic medical records (EMR) preferred.
• Ability to effectively coordinate multiple admissions and clinical priorities.
