What Is ACT?

Assertive Community Treatment (ACT) is a different way to practice nursing. Instead of a hospital floor or a clinic waiting room, our team goes to where our clients live — meeting them at home, in shelters, or wherever they need us. You'll work closely with a small, multidisciplinary team of prescribers, therapists, peer support specialists, and case managers who see the same roughly 100 clients week after week, which means you get to build real relationships and watch real progress, not just manage a rotating caseload. Position Summary

As the RN on our ACT team, you'll work alongside our team psychiatrist and APRNs to manage medication administration, deliver medical care, and support the broader treatment, rehabilitation, and recovery goals of the people we serve.


What You'll Do

  • Partner with the team psychiatrist and APRNs to manage the medication system, administer medications and medical care, arrange lab work, and keep documentation current
  • Monitor clients for medical issues and medication side effects, and coach both clients and teammates on what to watch for
  • Coordinate with pharmacies, medication reps, and outside providers to keep care seamless
  • Work with clients one-on-one to build strategies that help them stick with their medications — things like routines, reminders, and cues tailored to them
  • Make sure medications and injections go out on time, every time
  • Join daily team huddles and treatment planning meetings, where your clinical voice matters
  • Keep documentation current and meet ACT's minimum service standards

What We're Looking For

  • Current RN license in the state of Utah
  • Experience in inpatient psychiatric settings preferred (but we're open to strong candidates without it)
  • Experience working with individuals with serious mental illness and/or substance use disorders preferred
  • Sound clinical judgment and comfort working independently in the field
  • Strong organizational skills, attention to detail, and a self-starter mindset
  • Excellent communication skills and a steady hand in crisis situations
  • Basic computer literacy (Microsoft Office)
  • A collaborative, professional approach with clients and teammates alike
  • CPR and First Aid certification (or willingness to obtain)
  • Must be at least 21, hold a valid driver's license with a good driving record, and be insurable under the agency's liability policy
  • Must pass a Utah DHS Office of Licensing background screening and pre-employment drug screening

  • This role involves being on your feet and moving around throughout your shift, both indoors and out — including client homes, shelters, and other community settings.
