The RN Care Manager will provide a range of nursing and health care services to individuals with mental illness, co-occurring substance use disorders, children with serious emotional disturbance and/or all persons with intellectual/developmental disabilities under the supervision of the population team coordinator. Responsibilities include working with the prescribers and staff members to coordinate psychiatric evaluations and medication reviews; monitoring consumer response to medication treatment, administering prescribed medications to consumers; assessment, monitoring and therapeutic recommendations of psychiatric and physical health needs for consumers in various CMH programs. Services will be provided in the three-county area at the program site through such activities as: medication administration, group education to consumers or staff, individual consumer education, medication reviews, routine monitoring of consumer health needs as appropriate.

The RN Care Manager is also responsible for providing supports coordination and/or targeted case management services to individuals with mental illness, co-occurring substance use disorders, children with serious emotional disturbance and/or all persons with intellectual/developmental disabilities under the supervision of the population team leader or supervisor. These services may be one or more of the following: collaborating on service planning utilizing person centered planning principles, linking and coordinating to CMH and community services and community agencies, monitoring for service effectiveness, consumer satisfaction and serving as a consumer advocate and coordination with the Medicaid Health Plan or other health care providers. Consumers may require long-term support and intervention to maintain or improve functioning and have compromising physical or interfering behavioral conditions. This position is part of a staff pool that serves to fulfill the agency’s Crisis Stabilization Service. This may require on-call hours including nights, weekends, and holidays.
