    Interim HealthCare is seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our growing team in Keene, New Hampshire.

    Position Details

    • Location: Keene, NH
    • Schedule: Monday-Friday | 8:30 AM - 5:00 PM
    • Employment Type: Full-Time

    What You'll Do

    As an RN Case Manager, you will coordinate and oversee the care of a dedicated patient caseload while providing skilled nursing services in the home.

    Responsibilities include:

    • Conduct comprehensive patient assessments and develop individualized plans of care.
    • Provide skilled nursing visits and case management services.
    • Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
    • Monitor patient progress and adjust care plans as needed.
    • Provide patient and family education to promote independence and improved health outcomes.
    • Complete timely and accurate clinical documentation.
    • Supervise and collaborate with interdisciplinary team members to ensure exceptional patient care.
    • Advocate for patients while promoting quality, safety, and continuity of care.

    Qualifications

    • Current Registered Nurse (RN) license in the State of New Hampshire
    • Minimum of two (2) years of skilled nursing experience preferred
    • Previous nursing supervisory experience preferred
    • Recent wound care experience preferred
    • Current CPR Certification
    • Reliable transportation and valid driver's license
    • Strong assessment, communication, organizational, and clinical decision-making skills
    • Compassionate, patient-centered approach with a commitment to delivering exceptional home healthcare
