Elite Care Management- Indiana office, is seeking a dynamic RN Floater / Case Manager to join our team. This role is ideal for a nurse who thrives in a flexible environment, enjoys variety, and is looking to expand both clinical and leadership skills.
Position Overview
As an RN Floater / Case Manager, you will be cross-trained across multiple patient cases, providing both hands-on skilled nursing care and case management oversight. This includes supporting coverage needs while ensuring high-quality, compliant patient care.
Pay Range: $80,000 - $82,000
Key Responsibilities
- Perform supervisory visits and routine skilled nursing visits
- Provide shift coverage as needed across assigned cases
- Manage and coordinate patient care plans in collaboration with physicians and interdisciplinary team
- Monitor patient progress and update care plans accordingly
- Ensure compliance with all clinical, documentation, and regulatory standards
- Serve as a clinical resource for field staff and caregivers

Requirements
- Active RN License (State of Indiana)
- Current CPR Certification
- Home health or case management experience preferred
- Strong clinical assessment and critical thinking skills
- Ability to adapt quickly and manage multiple patient assignments
- Oasis documentation preferred
