We are seeking a reliable and experienced Registered Nurse (RN) to provide in-home assessments and support clients throughout Anne Arundel County. This role is ideal for nurses looking for flexible, supplemental income while making a meaningful impact in patient care and compliance.

This position primarily supports required client assessments and care coordination in a home setting. Travel between clients is required.

Responsibilities

• Conduct in-home patient assessments and develop individualized care plans

• Monitor patient condition and document findings accurately

• Ensure compliance with state requirements and assessment timelines

• Communicate changes in patient condition to physicians and other appropriate healthcare professionals

• Coordinate care with physicians, caregivers, and interdisciplinary team members

• Provide education to patients and families regarding care plans, medications, and disease management

• Maintain accurate, timely electronic documentation

• Travel throughout Anne Arundel County to complete scheduled patient visits

Qualifications

• Active Registered Nurse (RN) license in the State of Maryland

• Home care, home health, hospice, or community nursing experience preferred

• Ability to work independently and manage a flexible schedule efficiently

• Strong assessment, communication, and documentation skills

• Reliable transportation and willingness to travel throughout Anne Arundel County

• Commitment to providing compassionate, dependable patient care

Coverage Area

This position primarily serves clients throughout Anne Arundel County, including Annapolis, Crownsville, and Davidsonville, with occasional opportunities to assist in nearby communities as needed.
