    We are seeking a reliable and experienced Registered Nurse (RN) to provide in-home assessments and support clients across Maryland. This role is ideal for nurses looking for flexible, supplemental income while making a meaningful impact in patient care and compliance.

    This position primarily supports required client assessments and care coordination in a home setting. Travel between clients is required.

    Responsibilities

    • Conduct in-home patient assessments and develop care plans
    • Monitor patient condition and document findings accurately
    • Ensure compliance with state requirements and assessment timelines
    • Communicate changes in patient condition to appropriate parties
    • Coordinate care with physicians and other healthcare professionals
    • Provide education to patients and families on care plans and conditions
    • Maintain accurate and timely documentation

    Qualifications

    • Active RN license in the State of Maryland
    • Home care or home health experience preferred
    • Ability to work independently and manage schedule efficiently
    • Strong communication and documentation skills
    • Reliable transportation and willingness to travel between cases
    • Commitment to reliability and responsiveness
