What You'll Do

As a Home Health RN, you'll provide compassionate, individualized care while managing your own patient caseload.

Responsibilities include:

  • Complete comprehensive patient assessments and develop individualized plans of care.

  • Manage your own caseload while coordinating care with physicians, therapists, social workers, and other interdisciplinary team members.

  • Perform skilled nursing visits, patient education, medication management, wound care, and disease management.

  • Evaluate patient progress and adjust care plans as clinical needs change.

  • Identify physical, emotional, social, and environmental factors impacting patient health.

  • Communicate significant clinical changes to providers and the care team.

  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.

  • Advocate for patients and families while promoting independence and quality of life.

  • Ensure compliance with HIPAA, Medicare Conditions of Participation, and agency policies.

What You'll Need
  • Current, unrestricted Registered Nurse (RN) license

  • Minimum of one (1) year of nursing experience in an acute care, home health, hospice, skilled nursing, or comparable clinical setting

  • Current driver's license, reliable transportation, and auto insurance meeting state requirements

  • Willingness to travel throughout the assigned territory

  • Associate Degree in Nursing (ADN) required; BSN preferred

  • Home Health or Hospice experience preferred, but not required

  • HomeCare HomeBase (HCHB) experience preferred

  • Ability to frequently lift, push, pull, and support up to 50 pounds
