Key Responsibilities
Clinical Assessment & Care Planning
- Perform initial Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Develop, implement, and update individualized Plans of Care (POC) in collaboration with physicians
- Conduct comprehensive patient assessments, including physical, psychosocial, and environmental evaluations
Patient Care & Case Management
- Provide skilled nursing care, including wound care, medication management, and disease education
- Serve as the primary case manager for assigned patients
- Monitor patient progress and adjust care plans as needed
- Ensure coordination of care with therapists, LVNs, aides, and other disciplines
Documentation & Compliance
- Complete accurate and timely OASIS documentation
- Maintain compliance with Medicare Conditions of Participation (CoPs) and state regulations
- Ensure all documentation meets agency, regulatory, and reimbursement requirements
- Utilize the agency’s EHR system effectively
Care Coordination
- Communicate regularly with physicians regarding patient status and orders
- Participate in interdisciplinary team meetings and case conferences
- Ensure continuity of care across all services
Patient & Family Education
- Educate patients and caregivers on diagnoses, medications, treatments, and disease management
- Promote safety, fall prevention, and independence in the home environment
- Provide clear instructions for ongoing care and follow-up
Qualifications
- Active Registered Nurse (RN) license in the State of Nevada
- Graduate of an accredited nursing program
- Minimum 1–2 years of nursing experience (home health preferred)
- Strong clinical assessment and critical thinking skills
- Excellent communication and organizational abilities
- Ability to work independently and manage a caseload
Preferred Qualifications
- Home health experience strongly preferred
- Experience with OASIS documentation and Medicare billing requirements
- Familiarity with home health EMR systems (e.g., Axxess, WellSky)
- Bilingual (English/Spanish) is a plus
Physical Requirements
- Ability to travel to patient homes within assigned territory
- Ability to lift, assist, and reposition patients as needed
- Ability to perform physical tasks such as standing, walking, bending, and reaching
Work Environment
- Field-based position providing care in patients’ homes
- Flexible scheduling based on patient needs and caseload
- Independent work with strong clinical support from leadership
