Position: Registered Nurse (RN) - Home Health

Pay Range: $50.00-$65.00/hr
Status: Full-Time
Coverage Area: Coachella Valley

What You'll Do

The Registered Nurse (RN) plans, organizes, and coordinates home health services while delivering high-quality, patient-centered nursing care. The RN uses strong clinical skills and community-based nursing experience to support individuals and families in their homes, ensuring safe, effective, and compassionate care.

  • Develop and initiate the patient's individualized plan of care and lead ongoing, interdisciplinary assessments in collaboration with the patient, their representative, and caregivers
  • Implement appropriate preventative, therapeutic, and rehabilitative nursing interventions as ordered by the physician and outlined in the plan of care
  • Provide thorough patient and caregiver education, including disease management, medication management, safety, and home-based care techniques
  • Identify discharge planning needs early in the care process and ensure all necessary steps and education are completed prior to discharge
  • Coordinate delivery of medical equipment, supplies, and additional services to ensure patients have what they need for safe, effective care at home
  • Supervise Licensed Practical/Vocational Nurses (LPNs/LVNs) and Home Health Aides (HHAs), ensuring care is delivered in accordance with regulatory and organizational standards

Requirements
  • Valid CA Registered Nurse License
  • 2+ years of nursing experience, public health nursing or home health experience preferred
  • Maintains a current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
