We are
growing and seeking an
experienced, compassionate Home Health Registered Nurse (RN) to join our team on a
per diem basis in the Inland Empire area!

Bella Terra is part of the

Key Responsibilities- Develop and implement individualized plans of care in partnership with patients, caregivers, and interdisciplinary teams.
- Provide preventative and rehabilitative nursing procedures as ordered by the physician.
- Deliver patient, caregiver, and family education and counseling.
- Prepare and maintain accurate clinical notes.
- Communicate effectively with physicians and healthcare team members regarding patient needs and changes in condition.
- Identify discharge planning needs and implement prior to patient discharge.
- Supervise Licensed Practical/Vocational Nurses (LPN/LVN) and Home Health Aides (HHA/CNA) as directed.
- Participate in on-call duties, quality improvement programs, and in-service training.
Qualifications- Active California RN License
- Minimum 2 years of nursing experience (home health preferred)
- Current CPR certification
- Reliable transportation and valid driver's license with insurance
- Strong observation, problem-solving, and communication skills
- Ability to work independently and prioritize workload
- Physical ability to lift, transfer patients, and perform nursing duties
