    Registered Nurse (RN) – Home Health Visits and Start of Care (SOC) | Volusia County, FL
    Location: Volusia County, FL and surrounding areas
    Position Type: Home Health / Field-Based
    Schedule: Per diem and flexible visit opportunities
    Compensation: To be confirmed based on experience and visit type
    About Matrix Home Care
    Matrix Home Care is seeking compassionate, dependable, and experienced Registered Nurses to provide skilled nursing visits throughout Volusia County, FL and the surrounding areas.
    We are also seeking Registered Nurses with experience completing Start of Care assessments. This is an excellent opportunity for nurses who enjoy working independently, managing a flexible schedule, and providing individualized care to patients in their homes.
    Responsibilities
    • Complete scheduled skilled nursing visits in patients’ homes
    • Complete Start of Care assessments
    • Perform comprehensive patient assessments and monitor changes in condition
    • Develop, review, and follow the established plan of care
    • Complete OASIS documentation accurately and within required timeframes
    • Provide medication management and patient education
    • Perform wound care and other skilled nursing procedures as ordered
    • Communicate changes in the patient’s condition to the appropriate clinical team members
    • Complete accurate and timely clinical documentation
    • Maintain professional communication with patients, families, physicians, and interdisciplinary team members
    • Follow all agency policies, procedures, and applicable regulatory requirements

    Requirements

     Current and active Florida Registered Nurse license in good standing
    • Previous home health or skilled nursing experience is preferred
    • Start of Care and OASIS experience is strongly preferred
    • Ability to work independently in a home care environment
    • Strong assessment, communication, and documentation skills
    • Reliable transportation and the ability to travel throughout Volusia County
    • Valid driver’s license and current automobile insurance
    • Current CPR certification
    • Ability to complete all required employment and compliance documentation
    Background Screening Requirement
    Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
