Position Overview

Matrix Home Care is seeking an experienced and dependable Registered Nurse (RN) to complete home health visits throughout St. Cloud and the surrounding areas of Osceola County.

The RN will provide skilled nursing services, complete Start of Care assessments, coordinate patient care, and ensure that all clinical documentation is completed accurately and on time.

Responsibilities

• Complete Start of Care assessments and follow-up nursing visits
  
• Perform comprehensive skilled nursing assessments
  
• Develop, implement, and update individualized plans of care
  
• Provide wound care, medication management, and other ordered treatments
  
• Monitor patients for changes in condition and report concerns promptly
  
• Educate patients and caregivers regarding diagnoses, medications, treatments, and safety
  
• Coordinate care with physicians, families, therapists, and other members of the interdisciplinary team
  
• Complete accurate and timely clinical documentation
  
• Follow all agency policies, physician orders, and applicable state and federal regulations
  
• Promote patient safety, independence, and positive health outcomes in the home
  

Requirements

• Active and unrestricted Florida Registered Nurse license
  
• Current CPR or BLS certification
  
• Previous home health experience preferred
  
• OASIS and Start of Care experience strongly preferred
  
• Strong clinical assessment, communication, and documentation skills
  
• Ability to work independently in patients’ homes
  
• Reliable transportation, a valid driver’s license, and current automobile insurance
  
• Dependable, organized, and patient-focused
