Absolute Hospice, Inc., provides quality and passionate care to patients within the comfort and privacy of their home or place of residence. The Hospice RN establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other health care professionals by providing hands-on-care, on-going clinical management and education regarding hospice care. As a Hospice RN, you will educate patients and their caregivers on their conditions and the steps needed to ensure proper recovery. Additional responsibilities of our Hospice RN include: - Making initial nursing evaluation visit
- Continually evaluating the patient's needs
- Initiating, coordinating and revising the treatment plan
- Rendering nursing care and performing treatments, as ordered by the physician
- Participating in the Quality Assurance/Quality Improvement process
- Preparing accurate, orderly and timely reports and documentation
- Adhering to all practice standards as they apply to patient care
Requirements: - Caring, patient and flexible teacher with excellent communication and time management abilities
- Possess independent problem solving abilities, excellent clinical assessment skills
- Computer literate
- Good physical shape
- Ability to lift/transfer patients to and from wheelchairs, if necessary
- Associate's of Science degree in Nursing
- Valid Ohio RN license
- Minimum 2 years of nursing experience including hospice or home health experience preferred
- Valid driver's license and one year clean driving record
- Excellent writing, communication and charting skills
