Schedule: Per Diem — Every Other Saturday & Sunday
Responsibilities
As a Hospice Visit Nurse, you will:
- Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.
- Develop, implement, and update individualized plans of care based on patient needs.
- Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.
- Oversee and support the work of home health aides, social workers, and chaplains.
- Educate patients and caregivers to promote safety, comfort, and understanding.
- Maintain high standards of documentation, compliance, and clinical excellence.
- Contribute to continuous improvement efforts to elevate the quality of care across the agency.

Qualifications
- Active NJ Registered Nurse (RN) license
- Minimum 1 year of RN experience
- Willingness to travel within assigned service areas
- Valid driver's license, reliable transportation, and active auto insurance
- Strong communication and interpersonal skills
