Matrix Home Care is seeking experienced Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to support skilled nursing visits in the Davenport / Polk County, Florida area.<\/span>

Davenport is one of our growing referral areas, and we are looking to build a strong field nursing team to support patient care needs in the area.<\/span>
<\/p>
Position Summary<\/span>
<\/p>
The RN/LPN will provide skilled nursing services to patients in their homes, complete visits as assigned, support care coordination, and ensure accurate and timely clinical documentation. This role is ideal for a nurse who enjoys working independently in the field, providing compassionate care, and supporting patients in a home setting.<\/span>
<\/p>
Provide skilled nursing visits in accordance with physician orders and the patient’s plan of care<\/span>
<\/p>
Complete Start of Care, evaluation, follow -up, and routine visits as applicable by license and scope of practice<\/span>
<\/p>
Monitor patient condition and report any changes to the clinical team<\/span>
<\/p>
Assist with medication management, wound care, patient education, and other skilled nursing needs<\/span>
<\/p>
Complete accurate and timely clinical documentation<\/span>
<\/p>
Educate patients and families on disease management, medications, safety, and home care needs<\/span>
<\/p>
Coordinate with the clinical team to support continuity of care<\/span>
<\/p>
Maintain compliance with home health standards, company policies, and regulatory requirements<\/span>
<\/p><\/div>
Active Florida RN or LPN license in good standing<\/span>
<\/p>
Home health experience preferred<\/span>
<\/p>
Experience with skilled nursing visits preferred<\/span>
<\/p>
OASIS / Start of Care experience preferred for RN candidates<\/span>
<\/p>
Strong clinical assessment and documentation skills<\/span>
<\/p>
Ability to work independently in the field<\/span>
<\/p>
Reliable transportation and valid driver’s license<\/span>
<\/p>
