Job Summary
"Navitas Healthcare, LLC" is a compassionate and skilled Registered Nurse (RN) to provide high-quality care in a Long-Term Care (LTC) unit within a rehabilitation hospital setting. The RN will be responsible for delivering patient-centered care, coordinating with interdisciplinary teams, and ensuring safety and clinical excellence across all assigned duties.
Key Responsibilities
- Provide comprehensive nursing care to residents, including assessment, planning, implementation, and evaluation of care plans
- Administer medications and treatments as prescribed by physicians
- Monitor, document, and report patient conditions, vital signs, and clinical progress
- Collaborate with interdisciplinary healthcare teams to ensure coordinated, patient-centered care
- Ensure compliance with facility policies, procedures, and state regulations
- Educate patients and families regarding care plans and health maintenance
- Participate in quality improvement initiatives to enhance patient outcomes
- Maintain accurate and timely clinical documentation
Shift & Schedule
- Day Shift (variable scheduling)
- 12-hour shifts
- 36 hours per week
- Immediate start available
Qualifications
- Active Registered Nurse (RN) license in New Hampshire
- Experience in Long-Term Care or rehabilitation preferred
- Strong clinical assessment and critical thinking skills
- Ability to work in a fast-paced patient care environment
- Excellent communication and teamwork abilities
