Location: Berlin, NH
Facility Type: Rehabilitation Hospital / Long-Term Care
Shift: Day Shift (Variable 12-hour shifts)
Schedule: 36 hours/week (ASAP Start)


Job Summary

"Navitas Healthcare, LLC" is a compassionate and skilled Registered Nurse (RN) to provide high-quality care in a Long-Term Care (LTC) unit within a rehabilitation hospital setting. The RN will be responsible for delivering patient-centered care, coordinating with interdisciplinary teams, and ensuring safety and clinical excellence across all assigned duties.


Key Responsibilities

  • Provide comprehensive nursing care to residents, including assessment, planning, implementation, and evaluation of care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor, document, and report patient conditions, vital signs, and clinical progress
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated, patient-centered care
  • Ensure compliance with facility policies, procedures, and state regulations
  • Educate patients and families regarding care plans and health maintenance
  • Participate in quality improvement initiatives to enhance patient outcomes
  • Maintain accurate and timely clinical documentation

Shift & Schedule

  • Day Shift (variable scheduling)
  • 12-hour shifts
  • 36 hours per week
  • Immediate start available

Qualifications

  • Active Registered Nurse (RN) license in New Hampshire
  • Experience in Long-Term Care or rehabilitation preferred
  • Strong clinical assessment and critical thinking skills
  • Ability to work in a fast-paced patient care environment
  • Excellent communication and teamwork abilities
