  What You'll Be Doing<\/span><\/b>
  <\/p>

  • Provide skilled <\/span>home health nursing visits<\/span><\/b> throughout the Twin Cities service area<\/span>
    <\/li>
  • Complete comprehensive patient assessments<\/span>
    <\/li>
  • Perform <\/span>wound care<\/span><\/b>, medication management, injections, and other skilled nursing procedures<\/span>
    <\/li>
  • Educate patients and caregivers on treatment plans and disease management<\/span>
    <\/li>
  • Coordinate care with physicians and interdisciplinary team members<\/span>
    <\/li>
  • Provide coverage across territories to support patient needs and growing census<\/span>
    <\/li>
  • Complete accurate documentation using <\/span>Homecare Homebase (HCHB)<\/span><\/b>
    <\/li>
  • Help ensure continuity of care while delivering exceptional patient experiences<\/span>
    <\/li><\/ul>

    Who We're Looking For<\/span><\/b>
    <\/p>

    • Active Minnesota Registered Nurse (RN) license<\/span><\/b>
      <\/li>
    • Experience in <\/span>home health<\/span><\/b>, hospital, rehabilitation, skilled nursing, or acute care preferred<\/span>
      <\/li>
    • Strong assessment and critical thinking skills<\/span>
      <\/li>
    • Comfortable adapting to different clinical environments<\/span>
      <\/li>
    • Excellent communication and time management abilities<\/span>
      <\/li>
    • Reliable transportation with the flexibility to travel throughout the <\/span>Twin Cities Metro<\/span><\/b>
