Responsibilities:
• Provide direct patient care and support to the nursing team • Collaborate with interdisciplinary teams to develop and implement patient care plans • Conduct patient assessments, develop and implement individualized care plans, and monitor patient progress • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Participate in quality improvement initiatives and contribute to the development of evidence-based practice

Requirements:
• Current licensure as a Registered Nurse (RN) in the state of Connecticut • BSN degree or higher preferred • Minimum 1 year of experience in a hospital or healthcare setting • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment
