    Position Purpose

    The Saint Francis Hospital Outpatient Heart Failure Program provides care to our patients from our surrounding communities that are experiencing every stage and type of heart failure. The program has consistently achieved Gold Plus status in Heart Failure from the American Heart Association and Advanced Certification in Heart Failure from The Joint Commission. This certification recognizes hospitals that have achieved high standards for the treatment of heart failure patients. The Outpatient portion of the program is centralized through our outpatient clinic within Saint Francis Hospital. This clinic specializes in various degrees of heart failure with a focus on improving our patients' quality of life and optimizing and improving their disease process. There is a strong focus on providing guideline-directed medical therapy (GDMT) and implementing the newest evidence-based care. Clinic leadership will work with you to ensure you are supported during your orientation process and tailoring the process to meet individual needs.

    What you will do

    This position is for the role of the Disease Management RN. This RN provides heart failure disease management education and assists in managing our patients within the Outpatient Heart Failure Program. These patients will be managed short- or long-term depending on their disease process.

    • In this role, you will work closely with our advanced heart failure MD and advanced practice providers in managing patient medical issues as they arise and additionally, providing our patients with the resources necessary to obtain their critically needed medication therapy and care.

    • Within our Outpatient Heart Failure Program, you may provide infusion therapy for patients requiring diuresis in the clinic. You will also provide patients with appointments in clinic in collaboration with the APPs to provide medication guidance and disease management.

    • You will assist with in-basket inquiries within the electronic health record and necessary phone calls to patients and other providers within the ministry to assist in managing patients care. You will provide inpatients that are late day discharges with scheduled appointments and bedside education prior to their discharge from the hospital.

    • In this important position, you will be a part of a strong interdisciplinary team that includes MDs, APPs, RNs, and MAs. Your colleagues value teamwork and prioritizing quality care to our patients.

    Minimum Qualifications

    • Education: Graduate of an accredited nursing program.

    • Licensure: Current Licensure as a Registered Nurse RN in Connecticut.

    • Certification: BLS (AHA/ARC).

    Preferred Skills:

    • Education: Bachelor of Science in Nursing (BSN) degree preferred.

    • Experience: 2--3+ years of Cardiology experience preferred.

    • Certification: ACLS and Stroke Certification preferred.

    Work Schedule:

    • Full Time, 40 hours/week.

    • 4 10-hour day shifts, flexible schedule

    • There is a possibility that this role may also travel to Saint Mary's Hospital in Waterbury
