Position Title
Registered Nurse (RN) - Palliative Care Specialist
Liberty Hospital
Position Summary / Career Interest:
Responsible for coordinating an interdisciplinary approach to deliver palliative healthcare services to patients. Facilitates the clinical operations and activities of the program.
Work Hours: M-F 7:30 am - 4 pm Onsite (No weekends/Holidays)
Responsibilities and Essential Job Functions
Coordinate with the interdisciplinary team to complete palliative care consults including assessment of physical, psycho-social and spiritual needs.
Provide consultation and education regarding symptom management and pain control for palliative care patients.
Develop, plan, and coordinate patient, staff and community education regarding end-of-life issues, ethical issues and palliative care.
Facilitation of the palliative care team members, including team development, mentoring and skill building.
Serves as a liaison for patients/families, nursing staff, physicians, and other services; supports the patient/family in informed decision making.
Collaborate with Director on palliative care budget, review of performance measures, and use of staff resources proportional to activity and patient care.
Support maintenance, develop and maintain policies and procedures; assist in marketing of program.
Utilize quality improvement process to improve effectiveness of care.
Collaborate with hospice in program development, education and support.
Facilitate appropriate discharge planning and assist with identifying hospice referral.
Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
3 or more years recent clinical nursing experience.
Preferred Education and Experience
Master Degree Nursing with specialty in palliative care
Palliative care
educator/coordinator or manager experience
hospice experience
program development experience
Required Licensure and Certification
Licensed Registered Nurse (LRN) - Single State - State Board of Nursing
Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification
Certified Hospice and Palliative Nurse (CHPN) - Hospice and Palliative Credentialing Center (HPCC) APN with specialty in palliative care.
