Assurance Care & Support Services, Inc. is seeking a part-time or per-diem Registered Nurse to provide clinical oversight for our non-skilled home care and New Jersey DDD services.
The RN will be responsible for completing initial assessments and reassessments, developing and revising plans of care, reviewing medication documentation, updating Medication Administration Records based on current prescriber orders, conducting supervisory visits, training caregivers and DSPs, and monitoring individuals’ health and safety.
Responsibilities:
- Complete initial nursing assessments and required reassessments.
- Develop, review, and revise plans of care.
- Conduct supervisory visits in individuals’ homes or community settings.
- Review current medication orders and medication lists.
- Prepare, review, and revise MARs based on authorized prescriber orders.
- Review completed MARs for missed entries, refusals, omissions, medication errors, and other documentation concerns.
- Monitor changes in individuals’ health, functional abilities, and support needs.
- Provide training and clinical guidance to caregivers, CHHAs, DSPs, and administrative staff.
- Review progress notes, incident reports, health records, and other service documentation.
- Communicate health and safety concerns to physicians, families, guardians, support coordinators, and agency management, as appropriate.
- Participate in care conferences, service-plan reviews, quality-assurance activities, and record audits.
- Maintain accurate, timely, and confidential clinical documentation.
- Follow agency policies and applicable home care, DDD, HIPAA, and nursing requirements.
Our agency provides non-skilled home care and DDD services. This position focuses on assessment, reassessment, care planning, staff training, medication-record review, and nursing oversight. The RN will not provide routine skilled nursing services or independently prescribe, discontinue, or change medication orders.
Qualifications:
- Current and unrestricted New Jersey Registered Professional Nurse license.
- Graduate of an accredited nursing program.
- Current CPR and First Aid certification.
- At least one year of nursing experience preferred.
- Home care, developmental disabilities, community health, behavioral health, or long-term care experience preferred.
- Experience with assessments, reassessments, plans of care, and MAR reviews preferred.
- Strong documentation, communication, organizational, and time-management skills.
- Valid driver’s license, reliable transportation, and willingness to travel to service locations.
- Ability to complete all required background checks and agency onboarding requirements.
Schedule: Part-time or per diem with a flexible schedule.
Work Setting: Field visits and administrative responsibilities supporting individuals receiving non-skilled home care and DDD services throughout New Jersey.
