Registered Nurse (RN)

WCS Healthcare is partnering with a Community Health Clinic in Philadelphia, PA seeking an experienced Registered Nurse (RN) to join their Primary Care team. This is an excellent opportunity for a nurse passionate about population health, patient education, care coordination, and chronic disease management.

Schedule:

• Monday-Friday
• 8:00 AM - 5:00 PM
• No weekends

Responsibilities

• Serve as the practice-based nurse supporting Primary Care Providers (PCPs) in delivering high-quality patient care.
• Coordinate patient care, referrals, medication refills, diagnostic testing, and follow-up appointments.
• Manage provider patient panels and assist with closing preventative care and chronic disease management gaps.
• Educate patients on medications, treatment plans, and disease management.
• Conduct medication reconciliation and provide health coaching to support care plan goals.
• Monitor patient access and ensure timely follow-up care.
• Participate in daily interdisciplinary team huddles and collaborate with providers and care teams.
• Provide telephone and in-person triage services.
• Perform nursing procedures including wound care, phlebotomy, venipuncture, and IV therapy as needed.
• Support office workflow, point-of-care testing, and medication administration.
• Coordinate care for patients with chronic medical and behavioral health conditions.

Qualifications

• Active, unrestricted Pennsylvania Registered Nurse (RN) license.
• Active CPR/BLS certification.
• Minimum 3 years of nursing experience working with adult and geriatric populations.
• Experience managing patients with chronic conditions such as:
  • Diabetes
  • Hypertension
  • CHF
  • COPD
• Strong skills in wound care, phlebotomy, venipuncture, and patient assessment.
• Community health, primary care, ambulatory care, FQHC, or population health experience preferred.
• Excellent communication, organizational, and critical-thinking skills.
• Proficiency with EMR systems and Microsoft Office applications.
