Position Overview:• We are seeking a compassionate and experienced Registered Nurse (RN) to join an outpatient primary care team in the Ellenville, NY
• The RN will provide direct clinical support to patients and providers, assist with care coordination, patient education, clinical assessments, and routine nursing responsibilities in a community health center environment.
Responsibilities:• Perform nursing assessments and obtain patient medical histories and vital signs.
• Provide direct nursing care and clinical support to patients in an outpatient setting.
• Assist providers with examinations, procedures, and treatments.
• Administer medications, immunizations, and treatments as ordered.
• Provide patient education regarding medications, treatment plans, preventive care, and disease management.
• Coordinate follow-up care, referrals, testing, and other patient services.
• Monitor and document patient conditions and clinical information accurately in the electronic medical record.
• Communicate effectively with physicians, NPs, PAs, medical assistants, and other members of the care team.
• Support preventive health initiatives and chronic disease management.
• Maintain compliance with organizational policies, clinical standards, and regulatory requirements.
• Provide culturally sensitive and compassionate care to a diverse patient population.
Qualifications:• Active New York State RN license.
• Graduate of an accredited nursing program.
• Current BLS certification.
• Prior outpatient, primary care, community health, or FQHC experience preferred.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work effectively in a multidisciplinary team.
• Bilingual skills are a plus.
