A nonprofit services organization in Brooklyn, NY is seeking a licensed Registered Nurse (RN) to join its Primary Care team at its outpatient health center.
About the Opportunity:
- Shift: Day
- Schedule: Monday to Friday
- Hours: Standard business hours (rotates to 11am to 7pm every other week)
- Setting: Outpatient Health Center
Responsibilities:
- Provide direct nursing care, including patient assessment, triage, treatment, and medication administration
- Document nursing activities, clinical notes, and patient responses accurately and thoroughly
- Coordinate care with providers, nurses, medical assistants, operations staff, and external healthcare partners
- Support interdisciplinary care planning and participate in patient case conferences
- Maintain professional standards while caring for a diverse adult patient population in a community health environment
Qualifications:
- Bachelor's Degree in Nursing
- Active NYS Registered Nurse (RN) license
- BLS certification
- Experience in Primary Care or Emergency Room Nursing
- Experience administering medications and vaccines
- Ability to provide care for diverse and medically underserved populations
Desired Skills:
- Bilingual (English and Spanish)
- Experience in a Community Health Center or FQHC setting
- Background in Behavioral Health or Psychiatric Care
- Experience working with patients living with HIV/AIDS, Substance Use histories, or Mental Health needs
- Familiarity with Interdisciplinary Care Planning
