Registered Nurse (RN) - Primary Care

A health services network in New York City is actively seeking a licensed Registered Nurse (RN) with their Primary Care team in Manhattan.

About the Opportunity:

Responsibilities:

• Provide nursing care through assessment, planning, treatment, and documentation for a designated patient population
• Administer medications and vaccinations as prescribed and monitor patient response
• Perform patient triage and coordinate care with providers, nurses, medical assistants, and operations staff
• Maintain accurate clinical documentation, including medication profiles, nursing notes, and required records
• Support interdisciplinary care planning, patient case conferences, and continuity of care

Qualifications:

• Bachelor's Degree in Nursing
• Active New York State Registered Nurse license
• BLS certification
• Primary Care or Emergency Room experience
• Experience administering medications and vaccines
• Experience providing care to adults in a diverse patient population

Desired Skills:

• Bilingual (English and Spanish)
• Experience in a Community Health Center or FQHC
• Behavioral Health or Psychiatric Nursing experience
• Experience working with patients with HIV/AIDS, substance use histories, or mental illness
• Experience in Interdisciplinary Care Planning
