Key Responsibilities

• Assess residents' conditions and monitor for changes in health status.
• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
• Administer medications, treatments, and nursing interventions according to physician orders and facility protocols.
• Provide education, guidance, and support to residents and their families.
• Maintain accurate, timely, and thorough clinical documentation.
• Respond promptly and appropriately to changes in resident conditions and emergency situations.
• Collaborate with physicians, therapists, CNAs, and other healthcare professionals to coordinate quality resident care.
• Advocate for residents while promoting their safety, dignity, comfort, and quality of life.

Qualifications

• Current California Registered Nurse (RN) license in good standing.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced, team-oriented environment.
• Previous post-acute, skilled nursing, or long-term care experience is preferred but not required.
