    Description

    About the RoleOne Home Health is seeking an experienced home health nurse to support our Indiana team in a part-time capacity. Openings are available in Lake County, South Bend, and the surrounding areas. What You'll Do
    • Conduct ongoing observation, assessment, and documentation of patient symptoms and progress.
    • Perform initial home care visits and reassess patient needs and progress as required.
    • Collaborate effectively with interdisciplinary team members to ensure coordinated care.
    • Demonstrate proficiency in accurate and timely OASIS documentation.
    • Provide supervision and guidance to Home Health Aide (HHA) field staff as appropriate.
    • Adhere to all applicable Medicare regulations and compliance standards.
    • Promptly notify the physician of any changes in the patient’s condition.
    • Coordinate care with other disciplines to support comprehensive patient needs.
    • Complete all documentation and paperwork in a timely and efficient manner.
    • Educate patients and caregivers on disease processes, medications, care plans, and treatments in accordance with policies and procedures.
    • Deliver high-quality care to patients in their homes, following established policies and procedures.
    • Perform wound care, including assessment and treatment, as well as IV line care and maintenance.
    What You Bring
    • Graduate of an accredited school of professional nursing; Bachelor of Science in Nursing (BSN) or Public Health Nurse (PHN) degree preferred.
    • Current Registered Nurse (RN) license in the applicable state(s) of practice.
    • Minimum of one (1) year of experience in an acute care setting or equivalent clinical experience.
    • Valid driver’s license with a reliable, well-maintained vehicle; must meet state and/or agency insurance requirements and maintain a good driving record.
    • Current CPR certification required.
    • Strong communication and interpersonal skills.
