Elite Care Management- Indiana office, is seeking a dynamic RN Floater / Case Manager to join our team. This role is ideal for a nurse who thrives in a flexible environment, enjoys variety, and is looking to expand both clinical and leadership skills.

Position Overview

As an RN Floater / Case Manager, you will be cross-trained across multiple patient cases, providing both hands-on skilled nursing care and case management oversight. This includes supporting coverage needs while ensuring high-quality, compliant patient care.

Pay Range: $83,000 - $85,000

Key Responsibilities

• Perform supervisory visits and routine skilled nursing visits
• Provide shift coverage as needed across assigned cases
• Manage and coordinate patient care plans in collaboration with physicians and interdisciplinary team
• Monitor patient progress and update care plans accordingly
• Ensure compliance with all clinical, documentation, and regulatory standards
• Serve as a clinical resource for field staff and caregivers

Requirements

• Active RN License (State of Indiana)
• Current CPR Certification
• Home health or case management experience preferred
• Strong clinical assessment and critical thinking skills
• Ability to adapt quickly and manage multiple patient assignments
• Oasis documentation preferred
