REGISTERED NURSE (RN) – SKILLED ASSESSMENTS & HOME CARE

We are specifically looking for an RN with real skilled-care experience who is confident performing comprehensive patient assessments.

Love and Loyalty Home Care is expanding our skilled care department and seeking an experienced Registered Nurse (RN) to perform clinical assessments, assist with skilled admissions, and provide skilled nursing services in the home.

This position is ideal for an RN with experience in home health, skilled nursing, hospital care, rehabilitation, private duty nursing, or another strong clinical environment who understands how to assess a patient beyond basic vital signs and routine caregiving.

WHAT YOU’LL DO

* Perform comprehensive initial and ongoing skilled nursing assessments.

* Complete clinical intake assessments for incoming skilled-care patients.

* Review medical histories, diagnoses, medications, physician orders, hospital discharge paperwork, and treatment needs.

* Identify changes in patient condition and appropriate clinical interventions.

* Identify skilled nursing needs and contribute to appropriate plans of care.

* Provide skilled nursing services according to physician orders and established care plans.

* Maintain thorough, accurate, and timely clinical documentation.

* Communicate with physicians and other providers regarding patient needs and changes in condition.

* Coordinate with hospitals, discharge planners, patients, families, and the Love and Loyalty clinical team.

* Educate patients and families regarding medications, treatments, disease management, and ongoing care needs.

* Work directly with nursing leadership as we expand our skilled care program.

WHO WE’RE LOOKING FOR

Required:

* Active, unencumbered RN license in the state where services are provided.

* Strong hands-on clinical assessment skills.

* Skilled clinical nursing experience.

* Ability to recognize and appropriately communicate changes in patient condition.

* Excellent clinical documentation and communication skills.

* Ability to work independently and manage responsibilities professionally.

Strongly Preferred:

* 1–2+ years of clinical nursing experience.

* Home health experience.

* Skilled nursing, hospital, rehabilitation, or private duty nursing experience.

* Experience completing patient admissions or initial assessments.

* Experience reviewing physician orders and plans of care.

* Experience with EMR systems and electronic clinical documentation.
