As an Admissions RN for Crown Hospice, you won't just be caring for patients—you'll be guiding them and their loved ones through one of life's most sacred journeys. You'll often be the very first face they meet, helping families navigate the hospice admission process with compassion, confidence, and clarity. Your presence will be a source of peace, your hands a source of healing, and your heart a source of hope.

    What You Will Do:

    • Serve as the primary clinician for hospice admissions, completing comprehensive admission assessments and coordinating a smooth transition to hospice services.
    • Educate patients, families, and referral partners about hospice philosophy, services, and what to expect throughout the admission process.
    • Deliver compassionate, skilled nursing care wherever our patients call home.
    • Collaborate with interdisciplinary teams to coordinate patient care and facilitate effective communication among providers and team members.
    • Provide clinical, emotional, and educational support to families and caregivers.
    • Advocate for patients by providing education on their conditions, treatment options, and available resources.
    • Ensure timely, accurate documentation and coordinate admissions to provide an exceptional patient and family experience.
    • Work closely with a team of professionals committed to dignified care.
    • Advocate for comfort, respect, and quality of life—ALWAYS.

    Qualifications

    • Current RN license and clinical experience (hospice, palliative, geriatric, home health, or acute care preferred).
    • Previous hospice admissions experience is a plus, but we are willing to train the right candidate.
    • A calm presence under pressure, exceptional assessment skills, and the ability to build trust during life's most difficult moments.
    • Strong communication, organization, and time management skills.
    • Independence, integrity, and a willingness to collaborate with an interdisciplinary team.
