Responsibilities/Job Description:

Fairview Range is hiring a Registered Nurse (RN) Visit Nurse to join our Home Care and Hospice Team in Hibbing, MN.

 

The Home Care and Hospice RN Visit Nurse are responsible for providing compassionate, skilled nursing care to patients in their homes in accordance with physician orders, established nursing protocols, and the agency's policies. The RN visit nurse plays a key role in ensuring the health and well-being of patients through regular visits, assessments, interventions, education, and coordination of care. This role focuses on managing chronic illnesses, sometimes providing end-of-life care, and supporting patients and their families emotionally and physically. The nurse collaborates with healthcare teams to ensure high-quality, patient-centered care.

 

Schedule:This position will work 72-80 hours per two weeks.  The shift length will be 8-hours on the day shifts with rotating weekends and on call. The position works approximately 10 weekends per year, and 2-3 days of call each month.

As an RN Visitor Nurse, you will be responsible for: 

  • Conducting in-home nursing visits to patients in accordance with the plan of care under the direction of the case manager.
  • Administering medications, treatments, and therapies as prescribed by ordering providers.
  • Assessing symptom management needs and implementing appropriate interventions.
  • Providing end-of-life nursing care, symptom management, and comfort measures for hospice patients.
  • Responding appropriately to changes in patient condition and communicates findings promptly.
  • Providing emotional support to patients and their families during end-of-life care.
  • Communicating effectively with interdisciplinary teams to coordinate care and address any changes in patient condition.
  • Ensuring compliance with regulatory and organizational policies and procedures.
  • Assisting with admissions, recertifications, resumptions of care, and discharge visits as assigned.
  • Supporting continuity of care by following the established care plan and notifying the Case Manager of recommended changes.
  • Identifying patient/family knowledge/skill deficits and teaches accordingly to accommodate the family/patient needs related to illness, age, and ability to learn.
  • Educating patients and families/caregivers on disease process/management, medication management, treatments and procedures, home safety, infection prevention and other needs as identified.
  • Completing accurate, timely, and thorough clinical documentation in the electronic medical record.
  • Documenting assessments, interventions, patient response, education, and communication according to agency, State, Federal, Medicare, Medicaid, and accrediting body requirements.
  • Capable of functioning under pressure and in stressful situations.
  • Participates in ongoing informational and/or education activities related to home care and hospice
  • Possesses current knowledge of regulatory standards, policies and procedures.
  • Maintains productivity expectations and completes assigned visits efficiently.
  • Participates in orientation, competency validation, mandatory education, and continuing education.
  • Maintains professional relationships with patients, families, referral sources, and healthcare providers.
  • Uses agency equipment and resources responsibly.
  • Maintains confidentiality of patient information.
  • Performing other duties as assigned
  • Complying with all policies and standards



Required Qualifications

  • Graduate of an accredited RN program
  • Basic Life Support (American Heart Assoc or Red Cross) within 30 Days
  • Registered Nurse Current MN
  • Driver's License Current MN and auto insurance

Preferred Qualifications

  • Bachelor's degree in nursing
  • 1 year acute, long-term care, home care or hospice

 

Qualifications: $37.13- $52.39 Hourly
