    Job Description

    Are you a compassionate and dedicated nurse with a passion for helping patients navigate the transition from home health care to hospice? We are seeking a Transition Nurse to provide guidance, support, and expert care to patients and their families as they prepare for hospice care.     Key Responsibilities:

    • Coordinate and ensure smooth transitions for patients moving from home health services to hospice care.
    • Collaborate with home health clinicians to assess patients for hospice care eligibility and facilitate timely transitions.
    • Educate patients and their families about hospice care options, addressing concerns with empathy and understanding.
    • Conduct on-site informational sessions for patients, families, and caregivers, helping them understand the hospice transition process and available resources.
    • Provide education and training to the home health care team on hospice eligibility and appropriateness, ensuring staff can identify when hospice care is the best option.
    • Lead in-services and educational workshops in the community to raise awareness of hospice care and its benefits.
    • Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to address patient needs during the transition process.
    • Perform comprehensive assessments of patients' physical, emotional, and psychosocial needs during the transition.
    • Serve as a primary point of contact for patients and families, offering support, resources, and counseling during this critical time.
    • Maintain accurate documentation of assessments and communications in accordance with healthcare regulations.

    Qualifications:

    • Valid RN or LVN license in CA.
    • Minimum of 3 years of nursing experience, with a preference for home health, palliative, or hospice care experience.
    • Strong communication and interpersonal skills to provide compassionate care and education.
    • Ability to work both autonomously and as part of a multidisciplinary team.
    • In-depth knowledge of hospice and end-of-life care principles.
