Key Responsibilities:
Coordinate and ensure smooth transitions for patients moving from home health services to hospice care.
Collaborate with home health clinicians to assess patients for hospice care eligibility and facilitate timely transitions.
Educate patients and their families about hospice care options, addressing concerns with empathy and understanding.
Conduct on-site informational sessions for patients, families, and caregivers, helping them understand the hospice transition process and available resources.
Provide education and training to the home health care team on hospice eligibility and appropriateness, ensuring staff can identify when hospice care is the best option.
Lead in-services and educational workshops in the community to raise awareness of hospice care and its benefits.
Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to address patient needs during the transition process.
Perform comprehensive assessments of patients' physical, emotional, and psychosocial needs during the transition.
Serve as a primary point of contact for patients and families, offering support, resources, and counseling during this critical time.
Maintain accurate documentation of assessments and communications in accordance with healthcare regulations.
Qualifications:
Valid RN or LVN license in CA.
Minimum of 3 years of nursing experience, with a preference for home health, palliative, or hospice care experience.
Strong communication and interpersonal skills to provide compassionate care and education.
Ability to work both autonomously and as part of a multidisciplinary team.
In-depth knowledge of hospice and end-of-life care principles.
