Registered Nurse (RN) – Home Care AssessmentsTLC HomeCare Inc. is currently seeking a dedicated and experienced Registered Nurse (RN) to perform in-home patient assessments.
Responsibilities:• Conduct initial and ongoing patient assessments in clients’ homes
• Develop and review care plans based on patient needs
• Coordinate with caregivers and healthcare professionals
• Ensure accurate and timely documentation
Requirements:• Active RN license
• Experience in home care or clinical assessment preferred
• Strong communication and organizational skills
• Reliable transportation and willingness to travel
Schedule: Flexible
