Conduct initial and ongoing patient assessments in clients’ homes
Develop and review care plans based on patient needs
Coordinate with caregivers and healthcare professionals
Ensure accurate and timely documentationRequirements:
Active RN license
Experience in home care or clinical assessment preferred
Strong communication and organizational skills
