We are seeking a talented Registered Nurse to join our team. As a Home Health and Hospice RN, you will be responsible for planning, organizing, and directing home care services. You will have the opportunity to work in a community health setting, providing education and support to individuals and families within their homes and communities.
Job Summary:
The Home Health and Hospice RN plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Duties & Responsibilities:
Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
Provide preventative and rehabilitative nursing procedures as ordered by the physician
Offer patient, caregiver, and family counseling
Deliver patient and caregiver education
Prepare clinical notes
Communicate with the physician and other healthcare practitioners related to the home health plan of care
Identify discharge planning needs and implement prior to patient discharge
Communication:
Prepare clinical notes and update the primary physician when necessary
Communicate with the physician regarding the patient's needs and report any changes in the patient's condition
Coordinate care plan with community health-related persons
Additional Duties:
Participate in quality assessment and performance improvement programs
Participate in on-call duties as defined by the on-call policy
Ensure availability of necessary equipment and services
Instruct, supervise, and evaluate home health aide care provided every two weeks
Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed
Job Requirements:
Graduated from an approved school of nursing and licensed in the state of practice
Minimum of two years of experience, with preference for public health or home care nursing
Maintains a current CPR certification
Licensed driver with an insured automobile in good working order
Self-directed and able to work with minimal supervision
Excellent observation, problem-solving, and communication skills
Ability to organize and prioritize workload independently
No management experience required
Able to perform required nursing skills
