    Overview

    Job Duties

    Under the supervision of the Ambulatory Administrator or Nurse Manager, directs, coordinates, and controls the nursing activities of his/her assigned staff. Utilizes the Nursing Process to provide care for a group of patients. Assists the Medical Director or the Nurse Manager in evaluation of assigned staff. Assists in the provision of quality health care. Assignments are performed appropriately to age and developmental level of patients being served.

    Responsibilities

    Essential Functions

    • Supervise Exercise stress test. Helps set up telemedicine visits and patient education.
    • Accurately and efficiently documents findings, plans and interventions in the electronic medical record. Evaluates the effectiveness of care and documents subsequent changes to plan in the patient medical record.
    • Attempts to reduce readmission rate by counseling patient over phone and bringing patient in for in-person visit with Provider.
    • Counsels CHF patients and ACS patients on disease process.
    • Prescription refills (Cardiac patients on a great deal of medications).
    • Responds to all Patient Access Center task, patients issues. Triage medical issues.
    • Responds to physicians' task in EPIC.
    • Review all lab work, screening of lab work. Makes MD aware of abnormal test results
    • Works with pharmacist on medication issues. Identifies discrepancies and duplication of medication.
    • Peer to Peer counsel with Insurance companies.

    Qualifications

    Experience Requirements

    Experience in Cardiovascular field in hospital and ambulatory care setting required

    3 years RN Experience required

    Experience working with pre-post CT surgery patients and Pre-post Cath.

    lab and EP lab patients. required

    Education Requirements

    High School Diploma or GED required

    Bachelors Nursing preferred

    Certification/Licensure

    Registered Nurse (RN) - Florida Licensure required or at time of hire

    Registered Nurse (RN) - Georgia Licensure required or at time of hire

    Travel Required up to 25%
