Job Description: " Coordinates all aspects of care (inpatient and/or outpatient) - per program assignment, including but not limited to record review, medication refills and/or prior authorizations; patient education, review of test/procedure/lab results " Makes referral for medically necessary services, consults, equipment and supplies. " Functions as a liaison between administrative, clinical staff and physicians within the transplant department, as well as other departments involved with the care of patients.
CARE PROVISION: " Assists and supports patient discharge planning as needed " Provides patient education regarding care and medications " Provides assistance to patients and/or physicians in clinic and on hospital floors, as needed. " Carries out patient care orders as directed by physician and in accordance with hospital policy. " Assists with patient admissions and discharges as required
Requirements: BLS CA License Bachelor's EPIC Phoenix (strict requirement) Outpatient experience in EPIC.
Shift Details: M-F, 5x8s no weekends or holidays
