Position Purpose:
The Clinical Liaison works in coordination with the inpatient rehab facility team and networks including the appropriate physicians to: coordinate efforts within the admissions team to ensure timely response to all referrals, screen patients for appropriateness of admission to inpatient rehabilitation, communicate with the
admitting and consulting physicians, effectively speak with patients and family members throughout the evaluation process, as appropriate, facilitate admissions including required pre-authorization and pre-admission documentation. Collaborates with all network partners, and healthcare team to work toward the goal of problem solving as well as facilitating referrals to the appropriate level of rehabilitation.

Minimum Qualifications:
Education
Minimum: Associate's Degree in an allied health field from an accredited educational program.
Preferred: Bachelor's Degree from an accredited school.
Licensure/Credentials
Minimum: Licensed Practical Nurse, Occupational Therapist Assistant, or Physical Therapist Assistant license or other allied health licensure in the State of Michigan.
Valid driver's license with no restrictions
Related Experience
Minimum: Post-acute care experience.
Preferred: Experience in sales, and/or marketing. 2 years experience as a clinical liaison
