Rehab Patient Care Coordinator - (PT/OT/SLP)

Are you a licensed Physical Therapist, Occupational Therapist, or Speech-Language Pathologist looking to expand your impact beyond direct patient care? Join our growing home health team as a Patient Care Coordinator!

In this role, you'll help coordinate multidisciplinary patient care, collaborate with clinicians and office staff, ensure regulatory compliance, and support exceptional patient outcomes.

What You'll Do:• Coordinate and manage patient care services• Serve as a liaison between patients, families, providers, and caregivers• Manage referrals, authorizations, scheduling, and care plan coordination• Participate in interdisciplinary case conferences• Monitor patient progress and ensure continuity of care• Support quality assurance, chart audits, and regulatory compliance• Collaborate with clinical and administrative teams to improve patient satisfaction and outcomes

Qualifications: Active California PT, OT, or SLP License Current BLS Certification Home Health experience preferred Knowledge of Medicare regulations and OASIS preferred Strong organizational, communication, and computer skills Leadership, care coordination, or management experience is a plus
